The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

Our office accepts cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Please feel free to contact our office if you would like to discuss our financing option, Care Credit. Care Credit is a financing option that allows you to pay a low monthly payment and they even offer a no interest option. The application process is simple, an you can use Care Credit as soon as you are approved to finance your dental services. Now you don't have to worry about saving up for the procedures that you want and need.

Dental Insurance

As a service to our patient's, we accept most dental insurances. Our staff will prepare and submit all of the necessary information to your dental insurance company to ensure that your claims are paid. However, we remind you that your specific policy is an agreement between you and your insurance company. You will be responsible for your total balance if your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pretreatment estimate to your insurance company so that you will know what your benefits will be. The fees charged for services rendered to those who are insured are the usual and customary fees charged to all of our patients for similar services. Your policy may base it's allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available.
